1. Complete the Interest Inquiry through Google Forms. When you have successfully completed and submitted the form, you will see a confirmation message.
   

2. A member of the YESPhilly team will review your submitted form. If you qualify, we will contact you through email with additional application information and to schedule an interview to get to know you.
   

3. If you are eligible to enroll, you must:

   1. Complete the Interest Inquiry Form

   2. Complete the Interview

   3. Submit the Required Documents listed below

REGISTRATION CHECKLIST

• Identification: State ID, Birth Certificate, or Social Security card.

• Academic History: Transcripts and any existing IEP documentation.

• Health & Safety: Updated immunization records.

• Parental-Registration-Statement - MUST BE SIGNED BY PARENT IF STUDENT IS UNDER 18 YEARS OLD

• Proof of Address (only 2 of the following are required):

• Deed                                                                     

• Valid DOT Identification Card

• Current Utility Bill (gas, electric, cable, phone) 

• Current credit card bill

• Recent property tax bill                                        

• Recent vehicle registration

• Valid driver’s license/change of address card        

• Recent bank statement with address

• Letter from Social Security with address               

• IRS statement or wage and tax statement

• Letter from Public Assistance with address               

• Recent employer pay stub with address

• Foster care/childcare & DHs letters                          

• Shelter placement or residency letters

• Original Lease with name of parents/children

*Many of our participants face systemic hurdles in obtaining documentation. Our team provides active advocacy and support to help young adults navigate these requirements, ensuring that paperwork never stands in the way of progress.
